How do I create a survey?

The Survey block allows instructors to collect learner feedback and opinions through customizable surveys. Responses can be kept private to instructors or shared with learners, and can be anonymized to encourage honest participation. Five question types are supported: 

  • Short Answer and Long Answer for free-form text responses, 
  • Multiple Choice Single Answer and Multiple Choice Multiple Answer for selecting from preconfigured options, and 
  • Likert scale for measuring agreement across a standard five-point range.


  1. Navigate to the page and select Edit this Page. Find the position you want to add a block and select the plus (+) icon.

  2. From the dropdown, select Add Block.

  3. In the Add Block modal, either:
    • Use the Search field to find a block by keyword
    • Select a block from your custom list of Favorites
    • Scroll through the list of All Blocks to select one
    • Use the various Categories to filter by block type
    • Select a star icon to add or remove blocks in your Favorites listscreenshot of add block with the survey block showing

  4. Select the Survey block. The Add Survey form will open. The form has two areas: the editing pane and the preview pane. Changes made in the editing pane will appear in the preview pane, allowing the activity to be visualized as it is being built. There are three tabs in the Survey Block Form: Survey Settings, Add a Question, and Question Settings.

  5. Complete the Survey Settings with the following information:
    • Title: Enter a custom title for the survey. This title appears above the survey. The title will appear in the Overview page and the More Information area for learners and instructors. This field is required.
    • Points: Enter the number of points the activity is worth. This field only appears when the module is graded. The default value is 0. Enter 0 if you do not want to award any points for the activity. This field is required.
    • Visibility of Results: Select who can view the survey results after submission. This field is required.
    • Anonymous: Select whether responses will be associated with individual learners in the survey results. This field is required.screenshot of completed survey settings
  6. To build the Survey, add questions using the Add a Question tab. The following question types are supported: Short Answer, Long Answer, Multiple Choice Single Answer, Multiple Choice Multiple Answer, and Likert. Select the question type.

  7. The Question Settings tab will open with the selected question type displayed in the dropdown menu. Enter your question in the Question field. If using a Likert scale, the answer options are pre-populated—adjust them as needed. When all fields are complete, select Save Question.

  8. The questions will appear in the preview pane. Repeat until all questions in the survey have been added.

  9. The survey questions will appear in the preview pane. Carefully review the questions and use the pencil icon to make any necessary edits. Select the Add Block+ button to save the block to the page.Add survey edit form with Questions showing in the preview pane
  10. The new block will appear on the page in Edit Mode with a red, dotted outline indicating it's editable.

  11. Select the Publish My Edits button to save the block to the course page and see the content as it will appear to learners.

Adding a Multiple Choice Multiple Answer question type to the Survey block


For a simpler question format, consider using the Poll block. The Poll block presents a single question with up to five preconfigured response options, from which learners may select one answer.
